Target group
Systematic Early Detection (Triage) is a digital overview tool that helps frontline staff, in nursing homes and in home care, to detect early signs of illness in older citizens. The care staff quickly and easily records everyday observations about the citizen via smartphone/tablet and gets an ongoing overview of the citizen's condition on a digital overview board.
This way, nursing staff saves time on registrations and has more time for citizen-oriented care and management uses the solution to get a quick, dynamic overview of the conditions of several citizens and can thus prioritize efforts.
The product
Systematic Early Detection (Triage) uses very user-friendly observation tools – the so-called Roskilde Wheel and Change Schedule – which create a quick overview with just a few clicks.
The everyday observations have 6 categories, based on professional recommendations:

Each category has sub-items that are registered yellow or red depending on whether there has been a significant change in the citizen (triage). At the same time as triage, the nursing staff registers the level of condition and relations to the observation, thus avoiding having to report in more than one place.
When the nursing staff saves the record, the nursing management can immediately see it on their screen.
The data collected during the triage process is synchronized with the third-party electronic journal system. The overview boards display data from Pallas reports as well as data entered directly into the electronic journal system, along with information from other third-party systems, such as medication management and verified services.

The solution is intuitive and easy to learn and requires almost no training for new/changing employees. Frontline staff will therefore find that they spend less time on observation registrations, while being guided through a systematic and uniform citizen observation routine. This leaves more time for citizen-oriented care and attention. The graphical user interface and the built-in help options in the reporting ensure that temporary workers and staff with different cultural and linguistic backgrounds also use the same system and terms.
A quick overview of the citizen's current health status enables prioritized and rapid action. All knowledge is available to everyone in the team and requires professional discussion and exchanges, which qualify the citizen-oriented action. It also ensures a common basis of experience, which reduces misunderstandings and strengthens professional sparring in the team.
The above results in fewer hospitalizations and thus savings on municipal co-financing of hospitalizations and on nursing care after discharge.
