Systematic well-being and loneliness assessment of citizens with cognitive challenges
Target group
The tool is aimed at staff who work with citizens with cognitive limitations, including citizens with dementia-like behavior, in, for example, social housing/institutions and nursing homes. In short, all places where there may be a risk of developing loneliness and/or lack of well-being among citizens.
The product
Based on the recognized professional method for person-centered care – the Kitwood Flower, developed by the English psychologist Tom Kitwood – the tool helps staff to describe a citizen's well-being using 5 main areas: attachment, identity, employment, inclusion and comfort.
Registrations are collected in a well-being score, which is also displayed in green/red on constantly updated digital overview boards.
The module also includes a display of the initiated activities and the citizen's reactions to them.
Reports are generated on an ongoing basis so that developments can be followed and efforts can be initiated, evaluated, changed and possibly stopped.
Benefits
The solution identifies unmet psychological needs in citizens with cognitive limitations and enables early psychosocial interventions that can improve the well-being of both citizens and staff.
User-friendly graphical reporting is used for observations and it is possible to attach more details, so that even complicated processes can be described. The associated efforts/activities can be evaluated jointly.

Before an observation / measurement can be saved, all fields must be marked with either green or red.

The number of NO responses is shown as a number in a large red square.
The number of YES responses is shown as a number in a small green square.
If all questions are answered with a YES, it is displayed as a number in a large green square.
All registrations are automatically saved in the system, so that the history is visible and the citizens' development can be followed.
